Documents Required for Student Enrollment
There are several documents required for students to enroll in Gwinnett County Public Schools. A complete list is provided on the back of this flyer. IMMUNIZATIONS (#3231) AND SCREENING FORMS (#3300) USED FOR DAYCARE/PRESCHOOL ENROLLMENT WILL PROBABLY NOT BE VALID FOR KINDERGARTEN REGISTRATION. PLEASE CONTACT YOUR DOCTOR OR LOCAL HEALTH DEPARTMENT TO ENSURE YOUR CHILD HAS RECEIVED ALL REQUIRED IMMUNIZATIONS AND OBTAIN A CURRENT SCREENING FORM (CANNOT BE OVER A YEAR OLD).
All information must be complete for a child to be placed in a class and begin school.
NOTE: Proof of Residency Affidavits must be renewed each school year, include three notarized signatures(owner, tenant, parent), include
a Utility Bill with address of residence for the Tenant, and a piece of Business Mail with address of residence for the Parent.
Students transferring from another GCPS are required to withdraw in person from the previous school in order to enroll at the new school. Parents must obtain a
Withdrawal Form and a Change Of Status Form from the prior school including Proof of Current Health Records (State of GA Form 3231 and 3300). Parents not
having these documents will be required to return to the prior school for these documents in order to register and enroll at their new school.

It is strongly recommended that you register your child as soon as possible. If you wait until August, you will encounter long lines and possible delays. Please call for registration times on any teacher work day, as staff may not be available to complete your registration.
Registration for the 2018-19 school year will begin on Thursday, May 3, 2018. We will be using Gwinnett County’s new On-Line Registration System, so parents will be able to complete registration forms on-line prior to coming to registration. For a list of documents (ENGLISH or SPANISH) you will need to bring to the school to complete your child’s registration.
Register your kindergartner for the 18-19 school year by visiting GCPS on-line registration starting Thursday, May 3, 2018. Anything entered before then will be for the current school year.
